Maybe…you are hitting a mob/player with Pain Barrier?
This skill is really bad. Back in icbt2 i’ve tried it and its really bad. Single use, single target, delayed activation and can be resisted. The only use for it is to do ok damage on heavy armor target.
If they give it some changes, maybe a 3x cast or some aoe…
