Mathematically, Yes!
As long as it’s not 0%, he have a chance to not fail his Enhancement. And he got that chance.
Jealousy, Envy, Wrath, Greed, that chance will go straight 0% so it’s Nope.
But Mathematically, YES! It’s possible.

What does 7 sins have to do with what your saying?
Going +10 above gives you 50% chance of failing and you have to hit that 3x per successful upgrade… Thats like hitting the anvil 18x successful upgrade… That is almost 1/hundred millionth of a chance … The problem here is losing only 1 potential… It’s either he/she bought it from someone or it only lose 1 potential in the upgrade process.
Like I said, Mathematically, It’s possible.
Now, If you’re looking at it with Jealousy, Envy, Wrath and Greed, you will not accept the fact that it’s possible so you will deny all of the possibilities rendering yourself to think clearly.
I didn’t said that he didn’t used any other mean option.
As I said, as long as it’s not 0%, he have a chance to not fail his Enhancement. And he got it on this Enhancement.

Obviously an exploit being used; too many of these things are popping up. It’s possible, yes, but once multiple Lv220+ weapons upgraded to +15 or higher losing only 1 potential or none start popping up within the span of 1 month, then there’s more reason to believe it was done illegitimately over being upgraded fairly-- especially in this game with all the exploits it’s had since the server opened (Over here and on kToS).
Should be enough screenshots of weapons like these by now to get a GM to investigate to see what’s going on.

I heard there is an exploit that allows you to not lose potential by crashing channel. No clue how precisely it works and if its true.

It is possible , just that hes just super lucky 
I am not sure about +16 but i manage to +13 my superior corona rod only with 1 fail , the last few enhancement are about 51% which is a 50 50 chance but the rate is still higher by 1% , I did my enhancement infront of 15people in fedimian .
Took about like 3 superior corona rod till i succeed though , first 2 ended up as a failure with 2-3 fails so i just discard them , one of them manage to hit +11 with 1 fail before failing 3times in a row attempting to +12

As long as it’s not 0% it’s mathematically possible (you just make the wrong question), but probably not obtained in a legal way.
With the exploit being a thing nowadays you just have to bet for the worst. WIthout proof you can’t say it’s an exploit, just report on the forums and wait imc to “investigate” the issue
You might as well say everyone gets a chance to win the jack pot, but its like 1 over millions and millions chance to get that.
But over here, we see tons of it.
But it’s not 1 over millions and millions.
It’s 3 in 625 (0.48%) to get a +15 with no potential loss. 1 in 200 is ok odds.
A +16 with 1 potential loss is fine.
