They are designed that way, you can’t stack the same monster gem. It is not a bug.

they should make a notification about that that you can’t put the same gem skill on one item… whats the use of the gem if not working its useless gem will be destroyed when extracted… zzzzzzzzzzzzzzzzzzzzzzzzzzzz

While they should, you also should do a small research first before doing anything in this game, it is not completed, it lacks lots of info in-game, so be extra careful when thinking in do something.
