Every time I used this DG it did not work even though it was activated, according to video!
It’s a known issue, they broke now they can’t fix it. Same with attributes bug and these random dcs.

nothing new to be honest… just a visual bug (red message said fail)
At least the skill still works
The only way to know if it succeed/fail is to see the system message. It will write if the reincarnate is successful or have failed
But it happened to not give any message, to appear the icon and not to revive … 
Mine still displays the message though, check if you disabled system messages etc. This can be addon related though since i use:
This addon displays all system messages in a seperate chatbox
The Reincarnate debuff will always appear regardless of success/failure.
Snow Rolling also bug.
