it is impossible to explore Sanctuary map 100% without glitching to “secret” room with the shenanigans of Psycho3 with Teleport 15. There are no triggers no hidden quests that lead there.

On a different notice: portal to Revelation on the same map cannot be accessed in Mouse mode.

And lastly, mobs on that map hit way harder than they should (even mobs in 140 dungeon don’t hit THAT hard), especially when elite monster summons then on top of you. And they are mostly ranged mobs to boot that never miss.
It seems that map has been forgotten and not re-visited for quite a long time.
