When i add 12 points on CON in the stat simulator that show me 23 CON. But when i add 12 points on CON in the game it show me 23 CON.
The calculator jumps from 22->24 CON when you put that point in because every few points in a stat adds a free ‘bonus point’. The game will not show this bonus point until you actually confirm that you want to put the point in.
In that last screenshot of yours, if you actually confirm putting that point into CON, you should see your CON jump to 24 even though it says it is only going to be 23.
Unequip everything and re-check your natural stats.

Also you wont see the bonus CON awarded until you apply that 23rd point:
you have (19base + 3 bonus) and are about to get 4th bonus for 20th base stat.
If i see properly stat changes show that you will get 2 HP regen (99 > 101) which means that you will get your con.
I got 2 bonus points in game as a stat simulator… And in this picture i dont have equips on.
