Bug Description :
(letting us know what you were doing before, during, and after the bug happened will help us a lot)
Finished all the quest in Veja Ravine, Vieta Gorge and Cobalt Forest. Noticed that I am missing Torn Diary Part 1 after that continue checking inventory and saw that I was missing the Experiement Journal Chapter 2 and 4. Finished Dadan Jungle and Novaha a week ago. I got 100% for all maps and I am pretty sure I finished all the quests.
