found out it is a bug related to headbutt + invulnerability
It’s less of a bug and more of an undesired thing.
Sometimes boss monsters do an attack and they are invulnerable during that attack. If you headbutt or otherwise interrupt the boss monster so they skip the attack and it never completes, they don’t release their invulnerability until they do that attack again. It’s been this way forever.
