How to extract blessed gems from weapon?

Hi, I am in need of extracting some blessed gems from a 315 weapon, but idk if I lose the weapon or the gems when I do it. I have both the hammer and the spell rod, but im not sure how to do it.

I most definitely do not want to lose the weapon. Is there any way to not lose either?

No, you will lose your weapon. You can extract blessed gems at blacksmith on klaipeda or orsha.
