Game Crashes when I kill monsters

Game Control 4/Joypad/Mouse) : xbox 360 controller

Bug Description : Basically when I kill any monster at all my game crashes.

Steps to reproduce the issue :

  1. Hit monster
  2. Kill monster
  3. ???
  4. Game crash

I was having the same issue, and I noticed the problem was my xbox376 controller. Try killing a monster with the adapter disconnected

I am having the same issue singe yesterday evening.
I am using the keyboard as game control.

This has caused me to die many times already in tenet chapel. I have no idea what it might cause the bug. It has happened when I am trying to farm in more mob crowded areas.
