Date and Time : After the last maintenance patch.

Bug Description : Some time when you queue for a mission or a Dungeon and you found a party, you will got disconnect to the server selection page.
Then when you go back to the game and try to “Re-matching party” to get back to your party you just found, the game will show “Commander loadfail” and kick you out to server selection page again.
For the other side, your party, will be remain only 4 people in it for the entire dungeon/mission.
Steps to reproduce the issue : queue for a dungeon/mission, randomly happen.
