What makes you want Plague Doctor with this build? Chaplain scales a bit with SPR but is actually meant to be used with Dex and the skills don’t take SPR into their factoring as far as I can tell. Also if you’re going Chaplain and Monk, why not take Chaplain’s AA speed bonus as well as Monk’s to double down on that. If you go that route wouldn’t Priest be the better support pick over Plague Doctor? Then you’d get another attack on your auto attack, an extra 800-1000 holy dmg per attack (including that bonus to the extra attack Sacrament gives you), and much better heals than PD plus a defensive.

As for stats, STR and DEX would probably be good bets unless you need more hp.
Sacrament doesn’t stack with Last Rights…
