I have encountered this bug just this week after reset. Damage from unknown source 1-shots me.
From what I notice, it seems to happen whenever I get a debuff from mobs/boss. After the debuff’s duration, huge damage will come at me from out of nowhere. The damage color indicator is white, so it can’t be from mobs, it can crit as well. And I am not noticing any enemy that has the ‘Reflect’ skill before dying. Here is a video of the bug. Thanks!
It came from the monsters. It was the poison debuff.
I don’t know why it did so much damage, but yes, some monster poison debuff damage shows up as white. This is because it’s coded so that your character is inflicting damage to yourself rather than sourced from the enemy.
Even if it did, surely it is a bug. Even Moldyhorn in DS5 doesn’t 1 shot you with it’s OP poison, let alone a lv300 mob from stage 30 Bernice. There are also several unrecorded sessions where this happened. I know this is just Bernice but we do play it weekly and this bug is a huge waste of time. 
Its Luciferie, when you take freeze, poison, after you kill the source of the damage Luciferie Pyktis will proc on yourself.
I already had tested it with Lapasape Mushroom and died many times in CM’s
Have you also encountered this in Bernice? It is strange because i’ve had my accessories for quite some time and i’ve only encountered this problem this week.

M.izu definitely has it right. In this case, it’s just because one of those random monsters inflicted you with the type of poison that would cause this to happen (and/or they died). It should be a fairly rare occurrence, so it’s not surprising it only just happened now.

this bug has been in the game for a long time, when the source of the bleeding/poison dies it takes your stats and does damage base on that, the luciferi dmg is new too me, good to know.
IMC get your sh*t together and fix this please
