I used an anvil after defeating a boss and was removed from a quest instance because of my party member’s cutscene. Because I didn’t finish hitting the anvil, the item became locked in an enhancement state and bugged out.
It was fixed after logging out and logging in but the cost and anvil was lost for the item.
