It could be that you accidentally accepted a duel request from someone in your team, duel ended, and you with low HP died from the remaining mobs.

That, or you really had low HP already after the boss fight and died from leftover mobs.

Moreover, every time you enter a dungeon area the game keeps reminding you that items like gems will drop if you die.

Either you got trolled, was your fault, or both. No hacks at all.

Also, when reporting screenshots and/or video recordings would help.
